As you begin therapy :

  • No referral necessary! Let us take care of you. Our helpful front desk and intake staff take pride in arranging what is needed to get you started as quickly and efficiently as possible. If you do have a referral, we can have it faxed to our office or you can bring it in with you upon arrival.
  • Look at our Forms Section. We encourage you to fill out and submit the forms online if you are able. The forms can also be printed to fill out at your convenience and brought with you to your first appointment.
  • Please plan to arrive 20-30 minutes prior to your first appointment if you are unable to fill out your paperwork in advance.
  • Your first visit will be the initial evaluation. Please allow one hour for this appointment.
  • Following your evaluation, you will meet with the rehab receptionist to schedule future appointments and answer any insurance questions you may have.
  • Dress comfortably. Loose, comfortable clothing and tennis shoes are recommended. Please do not wear open toed shoes. Empty your pockets of change, keys or other bulky items before your appointment (we have cubbies available for your items).
  • You might be sore. And that is okay. You will be using muscles that have not been worked in a while so they may feel sore afterwards. This is completely normal and should diminish by the end of the first month. Chronic pain took a long time to develop and it will take longer to return to normal.
  • Be consistent with your exercises to achieve the best possible results.
  • Please be on time for your appointments. If more than 10 minutes late for an appointment, you may be rescheduled. 
  • Don’t get discouraged. Showing up is half the battle! Communicate with your therapist to let them know areas you are struggling with as well as successes you have achieved. This will allow the therapists to customize your program to improve your results.
